Parktree Community Health Center provides comprehensive and integrative medical, dental, and behavioral health services to the local community in the Pomona and Ontario areas.

We are looking for an energetic, professional Family Medicine Physician to join our team on a Full Time or Part Time basis.

Compensation:

• Full-Time Salary Range Family Medicine: $239,556.00 to $271,344.00 plus bonus eligibility
• Part-Time Salary Range Family Medicine: $115.17 to $130.45 per hour plus bonus eligibility
• Additional Compensation Opportunities:
• Earn up to an additional $27,000 bonus annually (prorated for part-time employees)
• Onboarding bonus of $5,000 after completion of 3 months tenure and successful completion of health plan credentialing

Responsibilities:

• Provides comprehensive, high quality and cost-effective primary care services to a diverse population in a patient and family centered environment. Within the course of a primary care visit, the physician will: Perform assessments, review of systems, physical examination; Develop a care plan; Prescribing treatments, therapies, medication and providing education on utilization and compliance; Refer to specialty care as is needed; Schedule follow-up appointments as needed; Assists patients/families in the development of self-management goals.
• Reviews results, signs-off, dates reports, and notifies patient/family of results of laboratory and diagnostic test results. Reviews transitions of care notes from hospital stays, emergency room visits and referrals to specialists.
• Prescribes appropriate immunizations in accordance with the Advisory Committee on Immunization Practices (ACIP) immunization schedule.
• In collaboration with the patient care team provides oral and written education regarding preventative and chronic disease management and recommends community resources to address the social determinants of health that might impact health outcomes.
• Functions on a rotating basis as the physician on-call during after-hours.
• Provides supervision for and signs off on Nurse Practitioner supervision and oversight for health plans.

Required Job Qualifications and Skills:

• Graduate of an approved medical school with a M.D. or D.O. degree.
• Current and unrestricted license in the State of California as a physician – Board eligible or certified
• Current DEA license /Current Basic Life Support certification (AHA healthcare provider approved course)
• Must be able to interact with people of various socioeconomic statuses and have the ability to address sensitive issues with tact and diplomacy
• Bilingual (English/Spanish)—Preferred, not required
• Experience in use of electronic health record
• Ability to work independently and/or in a team collaborative setting
